RN Case Manager – Outpatient

About the role
Key responsibilities & impact- responsible for the coordination of care for a complex patient population
- Perform ongoing telephonic case management activities
- Collaborates with patients, caregivers, and multi-disciplinary care team
- Develop, implement, evaluate, and revise case management care plans
- Identifies system issues that serve as barriers to care
- Promote member wellness and autonomy through advocacy and education
- Identify appropriate providers and facilities through the continuum of services
- Collaborate with case managers in discharge and transitional care process
- Stratifies and/or validates member level of risk
Requirements
What you’ll need- Registered Nurse with BSN required or relevant experience in Utilization Management or Case Management
- DE RN License or Compact Licensure
- Basic Life Support (BLS) certification required
- Case Management Certification preferred within 18 months of eligibility
- Advanced Certification recommended
