About the role

Key responsibilities & impact
  • Serve as the key point of contact and coordinator of care for members experiencing acute, catastrophic, chronic, or costly healthcare events
  • Identify, screen, and enroll eligible members into Care Management Services
  • Educate members and providers about Medi-Share provisions and processes
  • Confirm care eligibility using Medi-Share and FDA/CMS-approved evidence-based clinical management guidelines
  • Facilitate member stewardship, including medication, laboratory, and genetic testing assistance programs
  • Provide disease- and condition-specific clinical education
  • Use coaching skills, appreciative inquiry, motivational interviewing, and change management to improve clinical outcomes
  • Identify opportunities for and facilitate transitional care
  • Partner with organizational resources to achieve effective, cost-efficient member outcomes
  • Deliver accurate verbal and written communication while complying with HIPAA requirements
  • Document case work, calls, correspondence, and tasks in appropriate IT systems
  • Independently manage assigned caseloads and departmental workflows within service levels
  • Present case information to the Senior Director, Clinical Services
  • Complete medically related special projects and attend required meetings
  • Pray with members and providers as appropriate
  • Contribute to the expression of the Ministry’s Christian beliefs
  • Perform other assigned duties

Requirements

What you’ll need
  • Current unrestricted multi-state compact registered nurse license, or willingness to pursue the required license
  • 3+ years of nursing experience required; acute and catastrophic care highly preferred
  • 2+ years of case management and/or utilization management required
  • Bachelor of Science in Nursing preferred
  • Adult Oncology experience is a plus
  • Transitional care and transplant clinical experience highly preferred
  • Have or be willing to obtain case management or managed care nursing certification within two years if applicable
  • Expert knowledge of Medi-Share program guidelines and processes
  • Advanced Microsoft Office skills, including Word, Excel, and Outlook
  • Excellent computer skills and ability to use multiple software programs simultaneously
  • Keyboarding speed of at least 35 words per minute
  • Knowledge of HIPAA, PHI requirements, and call compliance rules
  • Strong research, analytical, critical-thinking, planning, and organizational skills
  • Ability to work independently and collaboratively
  • Ability to communicate effectively in written, oral, telephonic, and digital formats
  • Ability to use appreciative inquiry, motivational interviewing, and coaching skills
  • Ability to provide content within a biblical framework
  • Candidates must be at least 18 years old at hire
