About the role

Key responsibilities & impact
  • Provide high-quality, comprehensive social work services supporting healthcare and coordinating patients’ and families’ psychosocial needs
  • Complete comprehensive psychosocial assessments to identify strengths and potential barriers across the continuum of care
  • Assist patients and families in coping with illness, trauma, hospitalization, and end of life
  • Assist families and healthcare teams with discharge and ongoing medical care and treatment plans related to psychosocial needs
  • Help families access resources to achieve or maintain stabilization and self-sufficiency
  • Assess patients and families for safety, abuse, and neglect concerns
  • Serve as mandated reporter to Child Protective Services, Adult Protective Services, and law enforcement
  • Provide information and act as liaison for child protection, custody, supervision, visitation, and other safety needs
  • Document and maintain updated patient information regarding services provided

Requirements

What you’ll need
  • No minimum experience required
  • Preferred: experience in a medical setting and/or with children and families
  • Preferred: Licensed Master Social Worker (LMSW)
  • Preferred: Licensed Clinical Social Worker (LCSW)
  • Master’s degree in social work from an accredited graduate school
  • Must successfully pass the Basic Windows Skill Assessment at 80% or higher within 30 days of employment
  • Ability to work as part of an interdisciplinary team
  • Strong interpersonal skills
  • Knowledge of age-appropriate treatment and practice
  • Organizational and analytical skills
  • No professional certifications required
