About the role
- Make a POSITIVE IMPACT in your community by connecting with older adults
- Provide high quality in-home nursing care to older adults
- Collaborate with interdisciplinary team members to develop and implement individualized care plans
- Provide constant feedback to clients, client representatives, and nurses
- Work closely with the office team and use excellent communication and problem-solving skills
- Serve as the primary care manager coordinating care among RNs, therapists, and aides
- Coordinate care and act as primary point of contact for clients
Requirements
- Upbeat Personality
- Active California RN License
- Master in Nursing, preferred
- Industry Experience, preferred
- Strong Computer Skills
- Ability to multitask, drive, take action
- Strong Organizational & Communication Skills
- Pass Live Scan Fingerprint Clearance
- Valid driver’s license and proof of insurance required
- Cell phone with Internet Access
- Proof of Negative TB results
- Willing to Work One Weekend a Month as Back-Up On Call
