About the role
- Provide OT case management and support for older adults in home care
- Collaborate with interdisciplinary team members to develop and implement individualized care plans
- Provide constant feedback to clients, client representatives, and other healthcare providers
- Work closely with office team and communicate effectively
- Use problem-solving skills to address client needs and coordinate care
Requirements
- Upbeat Personality
- Active California OT license
- Industry Experience, preferred
- Strong Computer Skills
- Ability to multitask, drive, take action
- Strong Organizational & Communication Skills
- Pass Live Scan Fingerprint Clearance (LIC 9163)
- Valid driver’s license and proof of insurance required
- Cell phone with Internet Access
- Proof of Negative TB results
- Willing to Work One Weekend a Month as Back-Up On Call
