About the role

Key responsibilities & impact
  • Assess the needs, goals, and preferences of individuals with disabilities and special needs
  • Advocate on behalf of individuals and ensure connection to needed services and supports
  • Conduct annual and ongoing assessments to evaluate individual needs and service effectiveness
  • Develop and update individualized support plans with individuals and their support teams
  • Connect individuals to appropriate community resources and service providers
  • Monitor services for quality, satisfaction, compliance, and funding requirements
  • Advocate for individual rights, informed choice, and independence
  • Collaborate with families, providers, community partners, and internal teams to ensure coordinated care
  • Maintain accurate, timely documentation in accordance with compliance and regulatory standards
  • Uphold confidentiality, ethical standards, and professional boundaries
  • Travel to personal homes, residential provider locations, day programs, hospitals, schools, and detention facilities
  • Work under the general supervision of the Director of Individual and Family Support

Requirements

What you’ll need
  • Minimum age: 18
  • Bachelor’s degree OR RN licensure with 1 year of experience working with individuals with disabilities
  • Bachelor’s in unrelated field with 2 years of relevant experience
  • Strong communication, organization, and problem-solving skills
  • Proficiency in Microsoft Office and Therap
  • Valid driver’s license and Social Security number
  • Ability to type 70+ WPM and navigate basic computer systems
  • Basic computer and documentation skills
  • Ability to pass required background and reference checks
  • Frequent local travel throughout the Upstate region, across 12 Upstate counties
  • Reliable personal vehicle required
  • Ability to work independently with minimal supervision
  • Strict adherence to deadlines and non-flexible due dates
  • Regular standing, walking, lifting up to 25 lbs, and hands-on support
