About the role

Key responsibilities & impact
  • Demonstrates respect for individuals with disabilities and their support networks while understanding and adhering to the organization’s mission, goals, policies, and procedures.
  • Provide technical assistance to Providers implementing services for people referred
  • Lead, coach, and support a team of ICs through regular meetings, training, and performance management to maintain high-quality service delivery.
  • Advocate on behalf of person / guardian to assure that services being provided reflect the desires of person guardian.
  • Oversee person-centered planning and coordinated services by guiding staff, supporting transitions, collaborating with providers, and ensuring positive outcomes for individuals served.
  • Upon successfully transitioning to community services conduct 30, 60, 90 day follow up to assure satisfaction with services, and those desired outcomes are being achieved.
  • Maintain and provides other training as appropriate for supervised IC’s, person served, guardian, community providers.
  • Travel to and from assigned counties in Tennessee, as necessary.
  • Ensure compliance and quality by auditing case files and monitoring services.
  • Follow the standards and regulations required by Tennessee Department Disability and Aging (DDA).

Requirements

What you’ll need
  • At least 18 years of age
  • Must be able to effectively read, write, and communicate verbally in English
  • Ability to pass a background check upon hire
  • Valid driver's license
  • Vehicle Insurance
  • Master’s degree from an accredited college or university with experience in delivering or coordinating services and providing case management for individuals with disabilities and special needs. - OR - Bachelor’s degree from an accredited college or university in the human services field, or a related field; paired with one year experience in delivering or coordinating services and or providing case management for individuals with disabilities and special needs; in combination with proven supervisor or coaching experience.
  • Associate’s degree, a High School Diploma, or a GED, combined with four years of experience in delivering or coordinating services and or providing case management for individuals with disabilities and special needs; in combination with proven supervisory or coaching experience.
