About the role
Key responsibilities & impact- Triage after-hours phone calls and efficiently meet callers’ needs using excellent customer service
- Provide telephone advice and education for patient needs
- Handle telephone inquiries related to Chapters Health System’s services
- Promote and explain Chapters Health System’s services
- Gather necessary information and documentation for complete, current, accurate patient records
- Enter and track patient referral data in the EMR
- Reassure patients and families by phone
- Help resolve questions and identify when an in-person visit is needed
- Coordinate in-person visits when needed or requested
- Assist patients and families in achieving desired comfort levels
- Use support, expert resources, or personnel to resolve complex situations
- Document patient and family contacts in the EMR and communicate with the Interdisciplinary Group
- Report technical issues to leadership and resolve them within the required timeframe
- Report to a designated company location during emergencies when required
- Perform other assigned duties
Requirements
What you’ll need- Current Florida or multi-state license to practice as a registered nurse
- Ability to apply for or obtain a multi-state RN license within 30 days of hire if not already held
- Ability to apply for or obtain an RN license in Washington, D.C. within 30 days of hire
- Minimum of one year nursing experience
- Hospice or hospital experience preferred
- Previous experience with an EMR/EHR system
- Effective verbal and written communication in English
- Bilingual Spanish preferred
- Reliable transportation to meet patients’ needs
- Excellent critical decision-making and problem-solving skills
- Excellent clinical and assessment skills
- Ability to use equipment with visual and auditory mechanisms
- Excellent time management and prioritization skills
- Ability to handle stressful situations professionally and calmly
- Strong patient service, information-seeking, listening, and knowledge-transfer skills
- Ability to work in a fast-paced, cooperative team environment
- Ability to use interpersonal skills to maximize caller reassurance and patient visits
- Ability to exhibit urgency in daily work
- Computer proficiency, including MS Office, online research, data entry, and navigation
- Availability for shifts accommodating extended operating hours
- Ability to perform sedentary work with frequent interruptions
- Ability to manage emotional stress when caring for terminally ill patients and families
- Ability to perform essential physical functions, including lifting patients or equipment, bending, pushing/pulling, and kneeling, with or without reasonable accommodation
- Satisfactory completion of position competency requirements
- Consent to drug and/or alcohol testing after a conditional offer and ongoing compliance with the Drug-Free Workplace Policy
- Eligibility verification through the Florida Care Provider Background Screening Clearinghouse after a conditional offer and ongoing eligibility
