About the role
Key responsibilities & impact- Greet the patient and establish a relationship by explaining the Nurse Navigator role to patient/family/caregiver.
- Assess and identify the learning needs of the patient/family/caregiver and implement appropriate and effective teaching methods.
- Work closely with members of healthcare team to facilitate individualized plan of care for discharge/follow-up.
- Serve as the bridge between the patient and healthcare team by providing support and guidance to patient/family/caregiver.
- Collaborate with other staff regarding discharge calls and communication to transition to long term care, home nursing or Community Health Network.
Requirements
What you’ll need- Current Illinois Registered Nurse Licensure required.
- Minimum of 2 years nursing experience in acute patient care strongly preferred.
- BLS required within 30 days of hire and maintained throughout employment at CGH.
- Ability to develop and successfully implement teaching plan based on patients’ unique and individual patient needs strongly preferred.
- Ability to utilize computerized patient care management system (Cerner, Next Gen, MR View, and Midas) and ability to access outside sources as necessary for patient education strongly preferred.
- Basic Computer knowledge strongly preferred.
