About the role

Key responsibilities & impact
  • Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments
  • Develop and implement individualized care plans with physicians and other healthcare professionals
  • Monitor patients' conditions and report changes
  • Educate patients and families on disease management, medication, and treatment options
  • Maintain accurate patient care records and coordinate with healthcare professionals
  • Report patient care and condition progress to the patient's physician and Clinical Manager
  • Oversee Home Health Aides and monitor completion of their documentation

Requirements

What you’ll need
  • Med surg, ICU, ER, or acute experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Experience collaborating with a team of healthcare professionals
  • Valid driver's license
  • Auto insurance
  • Reliable transportation
  • One year of nursing experience preferred
  • Home Health experience preferred
  • Must complete TB screening if selected
  • Must maintain personal vehicle liability insurance according to state requirements or specified minimum limits
