About the role

Key responsibilities & impact
  • Provide skilled nursing care to patients in their homes under the direction of an RN
  • Implement individualized care plans with the patient, family, and healthcare team
  • Educate patients and families on disease management, treatment options, and self-care techniques
  • Maintain accurate records documenting patient visits, interventions, and outcomes
  • Communicate promptly with patients, physicians, clinical managers, and healthcare team members regarding needs and status changes
  • Participate in care coordination and discharge planning
  • Support patients in regaining independence while managing chronic conditions or recovering from illness, injury, surgery, or hospitalization

Requirements

What you’ll need
  • Current nursing license in the practicing state
  • Valid driver's license
  • Auto insurance
  • Reliable transportation
  • Current CPR certification
  • Two years of experience as an LPN/LVN in a clinical setting
  • Must complete TB screening if selected
  • Must maintain personal vehicle liability insurance in accordance with state minimums or specified higher limits
  • Home Health or Hospice nursing experience preferred
