About the role
Key responsibilities & impact- Develop, plan, organize, and supervise training for Clinical Care Nurses, care integration team members, and primary care providers
- Deliver education on population health, value-based care, senior care, STARS quality metrics, readmission risk, chronic disease management, medication reconciliation, caregiver burnout, advance care planning, and suicide prevention
- Incorporate EMR and systems training, compliance, policy and procedure, coding and documentation, quality, utilization, customer service, team effectiveness, and communication training as needed
- Deliver training in person in markets and virtually through live and recorded sessions
- Serve as faculty for clinical staff learning journeys and provider onboarding
- Teach transitions of care, care plans, 5M geriatric best practices, and motivational interviewing
- Monitor medical performance through observation, call listening, chart review, and data review; provide coaching and performance-improvement guidance
- Develop and complete scorecards documenting Clinical Care Nurse work quality and share data with stakeholders
- Support preceptors and the preceptor program during onboarding transitions
- Ensure licensed RNs meet competency requirements and assign education modules, shadowing, and hands-on work
- Collaborate with Regional Directors, Program Delivery, Compliance, market teams, and PCO leadership to identify training needs
- Represent the organization at recruiting and marketing events as needed
- Maintain active clinical practice in the local market for approximately 10% of the time
- Interview future RN hires as a subject-matter expert
- Serve as faculty for continuing-education sessions or CIT Grand Rounds as needed
- Travel up to 20% for onsite training delivery, role observation, and clinical practice
Requirements
What you’ll need- RN license in home state
- Willingness to obtain licensure in additional states as needed
- Minimum of five years’ experience working as a Registered Nurse
- Knowledge of community health and social service agencies and resources
- Experience working as part of an interprofessional team
- Experience in training and/or training content development
- Proficiency in Microsoft Office, EMR systems, and Zoom
- Familiarity with state Medicaid guidelines and application processes preferred
- Experience working with the geriatric/senior population preferred
- Experience in care management program management or implementation preferred
- Knowledge of Salesforce and AthenaOne preferred
- Knowledge of Team STEPPS evidence-based standards preferred
- Ability to reside in an existing PCO market
- Ability to travel up to 20%, largely within local and regional markets
- Minimum home internet speed of 25 Mbps download and 10 Mbps upload
