About the role
Key responsibilities & impact- Evaluate pharmacotherapy suitability for high-risk patients with chronic conditions including diabetes, hypertension, heart failure, and COPD
- Execute clinical protocols permitted by the organization and state regulatory agencies
- Optimize drug coverage benefits through formulary products, lower-cost alternatives, and home delivery pharmacy
- Collaborate with physicians, nurses, and healthcare professionals to optimize patient care
- Participate in multidisciplinary high-risk rounds and adjust treatment plans
- Counsel patients and communicate complex medical information to patients and providers
- Educate patients on disease states, medication use, and self-management
- Participate in medication safety and efficacy quality-improvement initiatives
- Perform post-discharge medication reconciliation for high-risk transition-of-care patients
- Document clinical activity in the clinic EMR and required pharmacy or clinical management systems
- Support CMS Stars and HEDIS quality measures
- Maintain documentation of clinical interventions and patient care activities
- Conduct in-service education and training for healthcare staff
- Develop and share pharmacotherapy educational materials
- Stay current with pharmacotherapy advancements and clinical guidelines
- Consult healthcare providers on complex medication management issues
- Provide support to the Primary Care Organization within the pharmacist’s scope of practice
- Implement targeted initiatives and priorities directed by Pharmacy leadership
- Report to the Associate Director Integrated Clinic Pharmacist
- Support the full physician panel across health plan coverage and potentially multiple clinic locations
Requirements
What you’ll need- Active and unrestricted pharmacist license in the state of Florida
- Willingness to become licensed in additional states for market support
- Pharm.D. or other advanced pharmacy degree
- Minimum five (5) years of experience as a practicing pharmacist
- Experience working in a team environment and supporting common goals
- Proficiency with Microsoft Word, Excel, PowerPoint, Teams, and Outlook
- Ability to troubleshoot and resolve basic technical needs in a remote environment
- Clinical residency in ambulatory care or similar discipline preferred
- Board certification in geriatric pharmacy preferred
- Experience in an ambulatory care/primary practice setting preferred
- Familiarity with electronic medical record (EMR) systems preferred
- Experience managing drug therapy for patients with chronic conditions preferred
- Ability to identify and implement innovative solutions for improved efficiencies and outcomes
- Multilingual ability, preferably English/Spanish
- Must reside within a 50-mile radius of Englwood Park, Orange City, West Oakland, West Orange, or Plant City
- Ability to work Monday through Friday, 8:00 a.m. to 5:00 p.m. Eastern
- Ability to travel approximately 5%, typically once quarterly for market meetings
- Home internet with at least 25 Mbps download and 10 Mbps upload speed
- Dedicated workspace without ongoing interruptions to protect PHI/HIPAA information
