About the role
Key responsibilities & impact- Plan and administer prescribed physical therapy treatment for patients in their homes to attain the highest level of physical function
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals
- Develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and patient-team goals
- Provide PTAs and Home Health Aides with written care instructions and oversee their performance and documentation
- Promptly and thoroughly document patient observations, interventions, and evaluations
- Report patient progress to the physician, Clinical Manager, care team, patient, and family
- Submit required evaluations, plans, progress notes, and discharge summaries
Requirements
What you’ll need- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid state driver's license
- Personal vehicle liability insurance meeting applicable state minimums or specified higher limits
- Preferred: One year of experience as a Physical Therapist
