About the role

Key responsibilities & impact
  • Provide non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness.
  • Guide members/families toward, and facilitate, interaction with community resources appropriate for the care and wellbeing of members such as housing, food stamps and other community assistance available.
  • Increase and maintain Dual Eligible membership in by utilizing appropriate reporting tools.
  • Assist patients with applying for additional services available through Department of Children and Families (DCF) such as Food stamps, government phones, and housing.
  • Addresses and documents inquiries pertaining to renewals or any new submissions by following department process and (DCF) guidelines.
  • Facilitate guidance/application for assistance available through government programs that patient could qualify for.
  • Follow up on case status.
  • Serve as an advocate and educator to applicants in need of assistance of services provided by DCF.
  • Comply with all HIPPA and rules and regulations.
  • Provide weekly and monthly report to direct supervisor.

Requirements

What you’ll need
  • Fully bilingual English and Spanish
  • Knowledge of Microsoft Word, Excel and Outlook
  • Able to learn and manage basic reporting tools
  • Administration support experience in a healthcare industry
  • Strong customer service skills
  • Detail oriented
  • Must be willing to travel locally to multiple Conviva centers.
  • Effective communication skills: both written and verbal.
  • Ability to multi-task, prioritize and manage time effectively.
  • Effective time management skills.
