Key responsibilities & impact

The Consultative Pharmacist 2 consults with patients, pharmacies and providers to provide patient care in the form of optimal medication therapy
Advises safe, accurate and timely usage of medications
Designs, implements, evaluates and makes adjustments to medication care plans
Monitors medication therapy and makes adjustments as needed
Works as the medication expert in focused proactive heath campaigns that target improving health outcomes for patients with specific conditions

Requirements

What you’ll need

Bachelors Degree
Pharmacist license in good standing in OH, AZ, Fl or TX
Interact with CenterWell Pharmacy patients via the telephone
Provide counseling and advice
Research patient concerns
Facilitate positive service and pharmaceutical outcomes
