Key responsibilities & impact

Ensure optimal continuity of care for patients transitioning into and out of services
Navigate healthcare options and services post-acute offerings
Handle a variety of customer issues including location lookup and complaints
Make clinical level of care determinations based on medical records
Act as a customer advocate throughout the referral process
Follow-up and track referral and admission outcomes
Provide education regarding Home Health, Hospice, and Palliative Care Services
Participate in special projects as assigned

Requirements

What you’ll need

Associates Degree required
BSN preferred
Compact state RN license required
At least 3 years post-acute nursing experience
Home health or hospice experience preferred
Knowledge of home health, hospice, and palliative care services
Must read, write and speak fluent English
Current CPR certification
