About the role
- Develops, assesses, and facilitates complex care management activities for primarily physical needs members
- Evaluates member needs, barriers to care, and social determinants of health; recommends/facilitates plans
- Develops ongoing care plans/service plans and collaborates with providers and community resources
- Identifies problems/barriers to care and provides appropriate care management interventions
- Coordinates between member/family/caregivers and care provider team to ensure person-centered care
- Provides ongoing follow up and monitoring of member status and revises care plans as necessary
- Provides resource support for local services (employment, housing, independent living, justice, foster care)
- Facilitates care management and collaborates with providers/specialists to ensure timely access to care
- Performs telephonic, digital, home and/or other site outreach to assess member needs
- Collects, documents, and maintains all member information and care management activities for compliance
- Provides education to members and families on disease processes, care gaps, referrals, and benefits
- Provides feedback to leadership on opportunities to improve care and quality delivery
Requirements
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 – 4 years of related experience
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
- Certified Diabetes Educator - Preferred
- Must reside in the state of Georgia
- Ability to perform telephonic, digital, home and/or other site outreach
- Ability to collect, document, and maintain member information to ensure compliance with regulators
