Assess and coordinate medical and supporting services across the continuum of care for complex/high acuity populations
Develop and continuously assess personalized long-term care/service plans and educate members and families/caregivers on services and benefits
Evaluate service needs and recommend plans for best outcomes, including assessments for higher level of care and waiver eligibility
Coordinate and manage communication between members/families/caregivers and care provider teams to ensure person-centered care
Monitor member status, changes in condition, and progress toward care plan goals; revise plans as necessary
Review member data, referrals, and intake assessments to develop appropriate care plans
Collaborate with healthcare providers to facilitate services and treatments and determine revised care plans
Collect, document, and maintain all member information and care management activities to ensure regulatory compliance
Provide and facilitate education on disease processes, care options, referrals, and healthcare benefits
Act as liaison and advocate between members/families, physicians, and facilities/agencies
Coordinate community resources and service authorization for services (meals, employment, housing, foster care, transportation, ADLs)
Perform home and facility site visits as required and travel to nursing facilities in Central Texas
Partner with leadership to improve quality of care and service delivery and may precept and coach new clinical hires
Requirements
Graduate from an Accredited School of Nursing or a Bachelor's degree
4–6 years of related experience
Bachelor's degree in Nursing preferred
RN - Registered Nurse - State Licensure and/or Compact State Licensure required
For Superior: Resource Utilization Group (RUG) certification must be obtained within 90 days of hire
Ability to travel to nursing facilities across Central Texas (Erath, Somervell, Bosque, Hamilton, Waco, Temple, Copperas Cove, Lampasas, Mexia, Freestone, Leon counties)
Availability Monday - Friday: 8 am - 5 pm (CST); no evenings, no weekends, no On Call
Ability to perform home and/or site visits as required
Experience in care management for complex/high acuity populations preferred
Experience precepting, coaching, and onboarding clinical new hires
