Uses a collaborative process of assessment, planning, implementing, coordinating, monitoring and evaluating options and services to meet an individual's health needs
Case management for high cost/high risk obstetric and neonatal or chronic conditions such as diabetes, hypertension, CHF, asthma, COPD, and/or depression (Excludes Behavioral Health)
Practices nursing within scope of licensure and adheres to policies, procedures, regulations, URAC standards and state regulations
Provides specialized education on disease specific conditions as needed
Serves as liaison among patients, families/caregivers, physicians, discharge planners, home care and ancillary providers
Works proactively with referrals to identify candidates for case management and maintains patient caseload in a timely manner
Requirements
Bachelor's degree in Nursing preferred
Registered Nurse (RN) with current active state license in good standing in the state(s) where job duties are performed required
Case management certification required (CCM, ACM, or RN-BC) or obtain URAC recognized within three years of hire
Minimum four (4) years' clinical practice experience in case management, home health, critical care, medical/surgical, discharge planning, concurrent review, or obstetric/neonatal care
Excellent oral & written communication skills
Detail-Oriented
Proficiency in Microsoft Office (Word, Excel, Outlook) and ability to navigate multiple systems
Ability to make sound judgements and decisions based on facts and guidelines
Ability to build collaborative relationships
Ability to interpret complex documentation
Ability to work independently with little supervision
