💰 $27 - $49 per hour
About the role
- Assists in developing, assessing, and coordinating holistic care management activities
- May develop or assist with developing personalized service care plans and educate members and their families/caregivers
- Evaluates member needs, resources available, and recommends plans for the best outcome
- Monitors care plans, member status, and outcomes as appropriate
- Interacts with long-term care healthcare providers to ensure member needs are met
- Provides education to long-term care members and their families/caregivers
Requirements
- Requires a Bachelor's degree
- 2 – 4 years of related experience
- Direct LVN or Service Coordination experience with adult populations (ages: 18 - 65)
- 3+ years of patient care coordination and managing medical and referral services
- Experienced LVN and/or Medical Case Managers preferred
