About the role
- Develops, assesses, and facilitates complex care management activities for primarily physical needs members
- Evaluates the needs of the member, barriers to accessing appropriate care, and recommends plan for best outcome
- Develops ongoing care plans and collaborates with providers
- Identifies problems/barriers to care and provides appropriate care management interventions
- Provides ongoing follow up and monitoring of member status
Requirements
- Two (2) years minimum of related professional pediatric care experience
- RN licensure required
- Must reside in Palm Beach County, FL
- Bilingual Spanish speaking candidates highly preferred
