About the role
- Assists in developing, assessing, and coordinating holistic care management activities
- Develop or assist with developing personalized service care plans/service plans for long-term care members
- Educates members and their families/caregivers on services and benefits
- Evaluates the needs of the member and recommends appropriate plans
- Assists with developing ongoing long-term care plans/service plans, identifying providers and resources
- Coordinates between the member, family/caregivers, and the care provider team
- Provides resource support to members and their families for various needs
- Monitors care plans/service plans and provides recommendations
- Interacts with long-term care healthcare providers and partners
- Collects, documents, and maintains long-term care member information
- Performs home and/or other site visits as needed
- Facilitates education to members and their families/caregivers on healthcare instructions and options
- Provides feedback on opportunities to improve and enhance quality of care
Requirements
- Requires a Bachelor's degree
- 2 – 4 years of related experience
- equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position
