About the role

Key responsibilities & impact
  • Provide education and information on available options, informed decision-making, and realistic goal setting
  • Establish, coordinate, review, evaluate, and revise supportive patient/caregiver care plans
  • Assist in developing comprehensive diagnostic and treatment plans through interdisciplinary input and weekly meetings
  • Manage and evaluate clinical data from initial referral through follow-up
  • Participate in clinical/patient rounds
  • Develop, coordinate, implement, and evaluate educational programs and projects for patients, caregivers, nursing staff, and support staff
  • Provide educational sessions for patients, caregivers, and clinical staff
  • Develop standards of care for patients undergoing new therapies and follow approved specialized nursing-care protocols
  • Track referral and transport volume and measure outcomes
  • Increase procedures, patient participation in clinical and research programs, and outreach activities
  • Recommend and implement policy/protocol revisions and performance-improvement activities
  • Assess educational needs and evaluate educational-program effectiveness and efficiency
  • Coordinate patient programs, projects, and events across Nursing, medical-center departments, and communities
  • Serve as an educational-program resource and consultant throughout the Cedars-Sinai Health System
  • Facilitate inter- and intra-departmental research and use clinical research findings to revise education programs
  • Create transfer agreements with local hospitals and build referral relationships with physician groups and hospitals in Southern California

Requirements

What you’ll need
  • Three (3) plus years of experience in an acute care setting, specifically transplant, cancer, breast, or pre-natal
  • Solid understanding of the Nursing Practice Act, The Joint Commission, and other state and local regulatory requirements
  • Solid grasp of organizational program or project coordination/support experience
  • Experience applying program/project management and development procedures
  • Bachelor’s of Science, Nursing (BSN) required
  • CA RN state License required upon hire
  • ANCC Specialty Certification required in area of specialty within 1-year of employment
  • Preferred experience with program development and implementation of navigator programs
  • Master’s degree preferred
  • Must currently live in the Los Angeles area or be willing to relocate to the LA area for onsite requirements
