About the role
Key responsibilities & impact- Manage the care and associated documentation of each assigned Soldier according to individualized treatment plans
- Track patients’ medical status and appointments
- Perform medical charting in eCase (MEDCHART)
- Request profile, evaluation, and audiology exams within required timelines
- Coordinate treatment plans with medical facilities
- Maintain automated referrals, medical documentation, electronic records, and hard-copy records
- Attend Medical Readiness events/PHAs as needed, at least annually
- Follow referred patients within seven calendar days
- Manage patient care from condition identification through medical readiness
- Apply Individual Medical Readiness and deployment regulations, including AR 40-501
- Obtain release-of-information documents and provide relevant documentation to records custodians
- Educate Soldiers on resources and lifestyle modifications supporting medical and dental readiness
- Maintain HIPAA standards and guidelines
- Review and track PHA/SRP lab results
- Upload, stamp, and index documents into the Health Readiness Record (HRR)
- Enter immunizations, pregnancy designations, eye exams, and medical warning tags in MODS
- Track attendance at medical appointments and follow-up documentation
- Log all service-member contacts and complete administrative charting in eCase
- Monitor compliance and run MEDPROS reports after PHA/SRP events
- Report medical readiness to commanders or designated representatives
- Case-manage Soldiers with T3, T4, P3, or P4 profiles
- Respond appropriately to urgent or emergent situations
- Maintain work areas, exam rooms, and supplies
- Attend meetings, prepare additional reports, and perform other assigned duties
- Follow client, employer, installation, safety, security, and traffic policies
Requirements
What you’ll need- Minimum high school diploma or GED certificate
- Familiarity with medical terminology and medical documentation practices
- Prior experience in patient care coordination activities
- Fully experienced and qualified in the relevant functional area
- Fluent English reading, comprehension, writing, and speaking ability
- Public Trust Clearance required
- Ability to pass a background check and any additional project/client checks
- High proficiency in Microsoft Office Suite
- Clear communication with patients in person and by telephone
- Adequate visual acuity for patient examination and reading printed and computer-based information
- Adequate physical ability and manual dexterity to perform healthcare services
- Ability to frequently bend, reach, perform repetitive hand movements, stand, walk, squat, and sit; regularly lift, push, and pull
- Problem-solving skills
- Ability to use computers and office productivity tools at sufficient speed
- Ability to work regular clinic hours, 7:00 a.m.–4:30 p.m.
- Ability to work flexible schedules, holidays, or outside normal hours when required
- Ability to travel for business or professional development when required
- Desired: experience with MEDPROS
