About the role

Key responsibilities & impact
  • Travel to patients' homes and provide healthcare services
  • Coordinate and provide direct and indirect patient care using the nursing process
  • Meet patients' and families' physical, psychosocial, environmental, and spiritual needs
  • Perform case management activities including assessment, coordination, planning, monitoring, implementation, and evaluation
  • Assess clients, caregivers, and families; plan care; arrange services; monitor progress; and provide support and education
  • Complete admission documentation and return documents to the office according to policy
  • Complete and timely transmit visit notes, telephone conversation records, physician orders, admission, routine, supervisory, telephone, and discharge notes
  • Ensure physician orders, care-plan corrections, verbal orders, and visit strings are documented and transmitted within required timelines
  • Complete home health aide supervisory visits
  • Coordinate the healthcare team and maintain linkages across the continuum of care
  • Initiate and document case conferences with other disciplines, physicians, care coordinators, and insurers
  • Provide care according to the plan of care and physician orders
  • Direct licensed practical nurse activities
  • Assign home health aides, prepare written instructions, and supervise aides in patients' homes
  • Perform OASIS assessments for admissions, recertifications, resumptions, and discharges
  • Perform complex wound care and central-line care
  • Access mediports for IV infusion or blood draws and flushes
  • Communicate patient updates and condition changes to physicians by phone or electronically
  • Manage assigned patient caseloads

Requirements

What you’ll need
  • Licensed Registered Professional Nurse (RN) — Illinois Department of Financial and Professional Regulation (IDFPR)
  • College diploma in Nursing required
  • 1 year of nursing experience
  • Basic Life Support (BLS) certification from the American Heart Association within 30 days
  • Illinois driver's license
  • Proof of auto insurance
  • Proficiency in interpersonal skills
  • Efficient time management
  • Ability to connect with patients
  • Ability to provide high-quality healthcare with minimal supervision
  • Knowledge of Medicare Home Care benefits
  • Knowledge of State and Federal regulatory and accreditation guidelines
  • Ability to perform OASIS assessments
  • Ability to perform complex wound care, including wound vacs, packing, and removal of staples or sutures
  • Ability to monitor central lines, including PICC and Hickman catheters, and perform dressing changes
  • Ability to access mediports for IV infusion or blood draws and perform flushes
