About the role
Key responsibilities & impact- Provide on-call coverage and perform patient visits as assigned and as needed for appropriate hospice care
- Provide visits and consultation to patients, families, and health care providers regarding hospice services and end-of-life treatment choices
- Perform the patient admission process
- Coordinate all patient care needs during evenings, nights, holidays, and weekend hours
- Maintain up-to-date patient records documenting problems, plans, actions, goals, and changes
- Provide patient and family visits based on assessed need, policy and procedure, and established schedule
- Follow established protocols for symptom management and patient visits
- Communicate the scope of home care or hospice care to patients, families, and health care providers
- Assess referrals for medical and environmental appropriateness for home care with the attending physician and interdisciplinary team
- Determine patient appropriateness for home care using established certification criteria
- Follow policies and procedures for admitting patients to Carle Home Care
- Coordinate delivery of medications, durable medical equipment, and medical supplies
- Record patient/family visits, telephone contacts, and referral actions in the medical record
- Document patient and family care clearly, concisely, and timely in accordance with policies, regulations, and accreditation standards
Requirements
What you’ll need- Licensed Registered Professional Nurse (RN) through the Illinois Department of Financial and Professional Regulation (IDFPR)
- Basic Life Support (BLS) certification through the American Heart Association within 30 days
- Illinois driver's license through the Illinois Secretary of State
- Proof of auto insurance
- College diploma in Nursing
- 1 year of nursing experience
- Must work Friday 4:00 p.m. through Monday 8:00 a.m.
- Weekend on-call availability every weekend
- Holiday-on-weekend coverage unless approved time off
