About the role

Key responsibilities & impact
  • Assist patients and their families with care issue resolutions via telephone, fax, and Epic in-basket communications
  • Coordinate delivery of care with physicians, therapy, and nursing disciplines
  • Provide education and instruction to patients and families by telephone
  • Perform patient care over telephone or Vidyo
  • Identify patient orders and prevent procedure delays to assure continuation of services
  • Complete supply ordering using the department formulary
  • Process open orders for clinical procedures
  • Manage telephonic visits to promote client health and manage cost per episode
  • Complete documentation and mandatory forms according to standards, policies, and procedures
  • Anticipate deteriorating life-sign patterns and changes in patient condition and take appropriate action
  • Document phone encounters accurately in the medical record
  • Use phone etiquette and service recovery methods to ensure customer satisfaction
  • Manage Epic in-basket communications between Home Health clinicians and physician offices
  • Notify the manager or designee of issues impacting workflow
  • Maintain accurate reports and records related to preventing and presenting complaints
  • Maintain cooperative working relationships with physicians, departments, staff, patients, and caregivers
  • Provide evidence-based clinical education to patients and representatives
  • Collaborate with patients to identify treatment-plan goals and encourage family participation
  • Assist clinical employees with knowledge of service excellence, Joint Commission, CMS, regulatory requirements, and clinical documentation
  • Coordinate additional patient visit needs with the scheduler
  • Collaborate with the intake team regarding information received by phone

Requirements

What you’ll need
  • Licensed Registered Professional Nurse (RN) through the Illinois Department of Financial and Professional Regulation (IDFPR)
  • College diploma in Nursing
  • 2 years of nursing experience
  • Knowledge of Epic in-basket communications
  • Ability to provide telephone or Vidyo patient care
  • Ability to document accurately in medical records
  • Knowledge of evidence-based clinical care practices
  • Knowledge of Joint Commission, CMS, regulatory requirements, and clinical documentation
  • Ability to coordinate care with physicians, therapy, nursing, scheduling, and intake teams
