About the role

Key responsibilities & impact

Serving patients of all ages through skilled home care nursing visits
Coordinating care plans, educating patients/family, and performing skilled nursing assessments
Performing complex wound care, wound vac, ostomy, PICC lines/Ports/Blood Draws, TPN and Antibiotic Infusion
Acting as the coordinator of the health care team to maintain proper linkages within the continuum of care
Performing case management for patients assigned to his/her case load

Requirements

What you’ll need

RN licensure in the state of Illinois
Experience in home health care preferred
Knowledge of Medicare Home Care benefits including covered services, recertifications, transfers, and non-recertifications
Experience with case management activities including assessment, coordination, planning, monitoring, implementation, and evaluation
Adequate knowledge of State and Federal regulatory and accreditation guidelines
