About the role

Key responsibilities & impact

Provide fast access to quality, compassionate after-hours RN telephone triage care to patients and caregivers of hospice and home health partners
Communicate with empathy and understanding, especially when callers are experiencing a difficult situation
Assess patient needs and provide appropriate care instructions
Coordinate care with other members of the healthcare team
Document patient care in the electronic health record (EHR)
Complete a brief skills assessment if selected

Requirements

What you’ll need

Must hold an active RN license in a compact state
4+ years of clinical nursing experience required
Experience in home health, palliative, med-surg, geriatrics, ICU, ER, and/or hospice care preferred but not required
English fluency is required
Spanish is a plus
Proficiency with computers and telecommunications equipment
Ability to work independently and as part of a team
Ability to work flexible hours, including evenings and weekends
Must be able to pass a background check and drug test for pre-employment screening
Must be able to meet California RN Required Prerequisites for RN Examination and Endorsements
Must have at least 400 work hours in the last two years
Personal computer with permitted internet connection (satellite broadband and mobile hotspots not permitted)
