About the role
- Perform a comprehensive psychosocial assessment of assigned consumers.
- Complete crisis assessments, initial assessments and integrated summary forms.
- Develop and implement a plan of care with ongoing consumer input.
- Provide individual mental health and/or substance abuse treatment and crisis assessment.
- Perform record keeping in accordance with Health Information Services and other Center requirements.
- Other duties as assigned.
Requirements
- Masters degree in Social Work or related field required.
- Candidate must have or be in the process of obtaining Independent license (LICSW and/or LMHC).
- Must have valid driver's license, registration, current inspection and insured automobile.
