Physiotherapist
Calgary West Central Primary Care NetworkPhysiotherapist assessing patients referred by physician members in Calgary. Providing self-management education and referral support within a hybrid work environment.
About the role
Key responsibilities & impact- Virtually assess patients referred by our physician members with a range of physical and functional concerns.
- Provide self-management advice, education, and referral/navigation support to CWC PCN contracted physiotherapy clinics and/or community programs.
- Gather and analyze information about the issue/concern from referral forms and through phone consultation with the patient.
- Interpret assessment findings and other relevant information.
- Work with the team’s program coordinator to book patients for screening and assessment.
- Interview patients to obtain relevant information about health conditions and personal and environmental factors.
- Determine the patient’s expectations and their relevance to physiotherapy.
- Develop physiotherapy diagnoses and plans for treatment.
- Refer patients to appropriate services based on assessment and eligibility.
- Provide self-management education to patients, as needed.
- Maintain accurate patient records in an Electronic Medical Record (EMR).
- Communicate referral outcomes and provide consult letters to referring physicians.
Requirements
What you’ll need- A bachelor’s or master’s degree in physiotherapy from an accredited university program is required.
- A minimum of five years’ clinical experience in a variety of settings (e.g., acute, community).
- The ability to manage a wide variety of clinical situations with varying levels of acuity, complexity, and diverse populations.
- A minimum of two years’ experience with the following is an asset: experience in the delivery of virtual or telerehabilitation interventions, experience with trauma informed care.
- Active registration with the College of Physiotherapists of Alberta is required.
- Certifications in diagnostic imaging and vestibular and concussion management are an asset.
- Proficiency in the Microsoft Office Suite (Word, Excel, Outlook, PowerPoint, Teams) and EMR systems (MedAccess, CHR).
- A clear Criminal Background check with Vulnerable Sector check is required.
- A current Health Care Provider CPR certificate (BLS Provider from the Heart & Stroke Foundation) is required.
