About the role

Key responsibilities & impact
  • Virtually assess patients referred by our physician members with a range of physical and functional concerns.
  • Provide self-management advice, education, and referral/navigation support to CWC PCN contracted physiotherapy clinics and/or community programs.
  • Gather and analyze information about the issue/concern from referral forms and through phone consultation with the patient.
  • Interpret assessment findings and other relevant information.
  • Work with the team’s program coordinator to book patients for screening and assessment.
  • Interview patients to obtain relevant information about health conditions and personal and environmental factors.
  • Determine the patient’s expectations and their relevance to physiotherapy.
  • Develop physiotherapy diagnoses and plans for treatment.
  • Refer patients to appropriate services based on assessment and eligibility.
  • Provide self-management education to patients, as needed.
  • Maintain accurate patient records in an Electronic Medical Record (EMR).
  • Communicate referral outcomes and provide consult letters to referring physicians.

Requirements

What you’ll need
  • A bachelor’s or master’s degree in physiotherapy from an accredited university program is required.
  • A minimum of five years’ clinical experience in a variety of settings (e.g., acute, community).
  • The ability to manage a wide variety of clinical situations with varying levels of acuity, complexity, and diverse populations.
  • A minimum of two years’ experience with the following is an asset: experience in the delivery of virtual or telerehabilitation interventions, experience with trauma informed care.
  • Active registration with the College of Physiotherapists of Alberta is required.
  • Certifications in diagnostic imaging and vestibular and concussion management are an asset.
  • Proficiency in the Microsoft Office Suite (Word, Excel, Outlook, PowerPoint, Teams) and EMR systems (MedAccess, CHR).
  • A clear Criminal Background check with Vulnerable Sector check is required.
  • A current Health Care Provider CPR certificate (BLS Provider from the Heart & Stroke Foundation) is required.
