About the role

Key responsibilities & impact
  • Serve as the primary clinical point of contact for patients navigating their remote care journey
  • Lead clinical decision-making for heart failure, type 2 diabetes, and hypertension using evidence-based guidelines
  • Manage a panel of patients receiving remote care and make timely clinical decisions
  • Initiate and titrate guideline-directed medical therapy while collaborating with physicians, RNs, and care teams
  • Respond promptly to clinical escalations from remote patient monitoring, including abnormal vitals, symptoms, and laboratory results
  • Provide synchronous and asynchronous care, including reviewing patient-generated health data, responding to alerts, secure messaging, and scheduled virtual visits
  • Meet productivity and documentation expectations while delivering safe, efficient, high-quality care
  • Adapt to evolving workflows, software enhancements, and operational processes
  • Use multiple technology platforms and provide feedback to improve clinical operations and technology
  • Collaborate across clinical and operational teams to improve patient care, efficiency, and scalability
  • Help build processes for a rapidly growing digital health organization

Requirements

What you’ll need
  • Active Compact Multi-State RN license
  • Active NP licensure in California, Washington, or Arizona, in good standing at time of hire
  • 5+ years of experience treating patients with chronic diseases, including CHF, hypertension, and type 2 diabetes
  • 3+ years of cardiology experience focused on heart failure management and guideline-directed medical therapy
  • Active ANCC or AANP certification
  • Master’s Degree as a Nurse Practitioner
  • Willingness to expand state licensure as Cadence adds new markets and partners
  • Home office setup with consistently stable Wi-Fi and strong upload and download speeds; Wi-Fi speed test required during interviews
  • Demonstrated success in fast-paced environments with frequently evolving priorities, workflows, and technology
  • Comfortable receiving frequent feedback and continuously adapting processes
  • Highly organized with excellent time management and ability to balance patient care with productivity expectations
  • Self-directed and accountable while working remotely with minimal supervision
  • Active clinical judgment when using AI-assisted clinical tools such as Gemini or Open Evidence
  • Experience in a CHF bridge clinic or T2D clinic, startup environment, and remote patient monitoring technology preferred
  • Availability Monday through Friday; preference for later weekday coverage, including until 11:00 PM Pacific Time
