About the role

Key responsibilities & impact
  • Serve as the primary clinical point of contact for patients navigating their remote care journey
  • Lead clinical decision-making using guideline-directed medical therapy for heart failure and evidence-based guidelines for type 2 diabetes and hypertension
  • Proactively and asynchronously titrate medications based on patient vitals, symptoms, and trends
  • Oversee patients, manage clinical escalations, and collaborate with physicians or providers to optimize treatment plans
  • Manage a panel of patients receiving remote care
  • Deliver synchronous and asynchronous care, including reviewing patient-generated health data, responding to remote monitoring alerts, secure messaging, and scheduled virtual visits
  • Initiate and titrate guideline-directed medical therapy while collaborating with physicians, RNs, and care teams
  • Respond to abnormal vitals, symptoms, and laboratory results
  • Meet productivity and documentation expectations
  • Adapt to evolving workflows, software enhancements, and operational processes
  • Use multiple technology platforms and provide feedback to improve clinical operations and technology
  • Collaborate across clinical and operational teams to improve patient care, efficiency, and scalability
  • Contribute to building better processes for a rapidly growing digital health organization

Requirements

What you’ll need
  • Active Compact Multi-State RN license, active and in good standing at time of hire
  • Active NP licensure in TX, active and in good standing at time of hire
  • 5+ years of experience treating patients with chronic diseases, including CHF, hypertension, and type 2 diabetes
  • 3+ years of cardiology experience focused on heart failure management and guideline-directed medical therapy (GDMT)
  • Active ANCC or AANP certification
  • Master’s Degree as a Nurse Practitioner
  • Willingness to expand state licensure as Cadence adds new markets and partners
  • Home office setup with consistently stable Wi-Fi and strong upload/download speeds
  • Must complete a Wi-Fi speed test during the interview process
  • Demonstrated success in fast-paced, frequently changing environments
  • Comfortable receiving frequent feedback and adapting processes
  • Highly organized with excellent time management
  • Ability to balance patient care with productivity expectations
  • Self-directed and accountable while working remotely with minimal supervision
  • Experience working in a CHF bridge clinic or T2D clinic, startup environment, and with remote patient monitoring technology preferred
  • Comfortable using AI-assisted clinical tools such as Gemini, Open Evidence, or comparable platforms
  • Clinical judgment must remain the primary guide for every patient decision
