About the role
Key responsibilities & impact- Assess patient eligibility for specific service needs
- Develop individualized integrated action plans
- Refer and connect patients to resources
- Monitor adherence and provide necessary follow-ups
- Assist patients in accessing medical, social, education, and employment resources
- Provide crisis and de-escalation techniques
- Ensure patients have reliable transportation to BrightView and other care providers
- Complete detailed reports on needs assessment and case management activities
- Enter necessary case management data into the electronic patient health record in a timely and comprehensive manner
- Attend required meetings, including supervision, treatment team meetings, and quarterly in-service
- Adhere to regulatory and company requirements in documentation and evaluation
- Participate in TCM audits and meet assessment and treatment plan completion expectations
Requirements
What you’ll need- 1 year of experience working with adults in a human service setting
- Bachelor's Degree in a related Behavioral Science field
- Excellent verbal and written communication skills
- Empathetic and compassionate approach to supporting patients' recovery
- Ability to work with a diverse population of colleagues and patients
- Problem-solving ability with a sense of urgency
- Ability to work collaboratively in a multidisciplinary healthcare environment
- Adaptability within a dynamic work environment
- Technological capability and comfort operating multiple communication and documentation systems
- Familiarity with MS Office software (Outlook, Teams, Word, etc.) preferred
- High degree of integrity and compliance with work standards and regulatory requirements
- Familiarity with HIPAA and 42-CFR Part 2 desired
- Prior experience with harm reduction a plus
