About the role
- Administers occupational therapy to patients on an intermittent basis in their place of residence
- Improve or minimize residual physical disabilities of the patient
- Return individuals to optimum and productive living within the patient’s capability
- Participate with all other home care personnel in patient’s care planning
- Provide prescribed occupational therapy and all skilled procedures
- Responsible for initial assessment, plan of care maintenance program development and modifications and reassessments every 30 days
- Consult with physicians regarding change in treatment
Requirements
- Graduate of an Occupational Therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and /or certified by successfully completing the National Certification examination, -or- Eligible for the National Registration Examination of the American Occupational Therapy Association
- Currently licensed by the Board of Occupational and Physical Therapy Examiners in the state in which practicing, unless licensure does not apply
- Minimum of one year in an acute care setting
- Two (2) years experience in Home Health preferred
- Access to reliable transportation
