About the role

Key responsibilities & impact

Coordinate and facilitate quality clinical management of a broad outpatient population across the continuum of care
Collaborate with providers on preventive care, evidence-based chronic disease management, and care coordination
Provide general and focused healthcare education and help patients establish self-management goals
Coordinate care with community providers and follow up on emergency department visits and inpatient discharges
Assist providers with examinations, treatments, and procedures
Provide telephonic triage, assessment, education, healthcare collaboration, and resource coordination
Perform direct patient care, including Annual Wellness Visits and Chronic Care Management
Manage INR and provide Coumadin teaching using a standardized protocol
Administer vaccines and medications according to provider orders and BIDHC medication policies
Perform nursing activities such as wound care and point-of-care testing; document patient care in the Electronic Health Record
Schedule tests and specialty-care referrals
Monitor and ensure follow-up of abnormal test and imaging results
Oversee clinical supply and equipment inventory and ordering
Ensure safe collection, storage, and processing of biologic specimens
Provide oversight, delegation, education, and training to MAs, Certified Nursing Assistants, Nursing Students, and practice staff
Maintain patient safety, protect PHI, and promptly report adverse events and near misses to Risk Management

Requirements

What you’ll need

Graduate from an accredited Nursing Program
Current Registered Nurse license in Massachusetts required
Basic Life Support certification required
CPR certification
1–3 years of related work experience required; new graduate RNs considered
Strong interpersonal, patient communication, and patient education skills
Advanced Microsoft application skills, including Outlook, Word, Excel, PowerPoint, or Access, and other web-based applications
Ability to communicate clearly and effectively in written English
Ability to comprehend and converse in English with staff, patients, families, and external customers
Ability to provide care to young adults, middle-aged adults, and elderly patients
Ability to work with infectious disease and bloodborne pathogen exposure precautions
Ability to perform medium physical work, exerting up to 50 pounds occasionally and/or 20 pounds frequently
Ability to stand, walk, bend, grasp, manipulate, push/pull, use a keyboard, and reach throughout the shift
