Key responsibilities & impact

Coordinate patient care for assigned caseload
Initiate and revise individual plans of care as patient status changes
Administer prescribed medications and treatments
Communicate with physicians regarding patient needs and changes in condition
Attend regular case conferences and team meetings
Complete and document required assessments and notes in a timely manner

Requirements

What you’ll need

Physical Therapist with current licensure to practice therapy in Massachusetts
Active American Health Association BLS
Graduate of an accredited program in Physical Therapy; DPT preferred
Minimum of 1-2 years physical therapy experience, with at least one in public health or home care preferred
OASIS experience is preferred
Must be a licensed driver with an insured automobile in good working order
Self-directed and able to work with minimal supervision
Strong ability to organize and prioritize workload independently
