About the role

Key responsibilities & impact
  • Coordinate patient care for assigned caseload
  • Initiate and revise individual plans of care as patient status changes

Requirements

What you’ll need
  • Physical Therapist with current licensure to practice therapy in Massachusetts
  • Active American Health Association BLS
  • Graduate of an accredited program in Physical Therapy; DPT preferred
  • Minimum of 1-2 years physical therapy experience, with at least one in public health or home care preferred
  • OASIS experience is preferred
  • Must be a licensed driver with an insured automobile in good working order
  • Self-directed and able to work with minimal supervision
  • Strong ability to organize and prioritize workload independently
