💰 $80,080 - $107,777 per year
About the role
- Plan, organize, collaborate, and direct home care services
- Coordinate care for assigned patient caseload
- Complete and document initial patient assessments
- Develop patient-specific plans of care
- Initiate and revise plans based on patient status changes
- Administer medications and treatments as prescribed
Requirements
- Degree from an accredited program in Physical Therapy; DPT preferred
- Current valid Massachusetts Physical Therapist license
- Active American Health Association BLS certification
- 2+ years of physical therapy experience, at least one of which is in public health or home care
- OASIS experience is preferred
