About the role
Key responsibilities & impact- Conducting comprehensive assessments for older people within the Aged Care Planning Regional area of Victoria (VIC 3 – Loddon Mallee)
- Delivering high-quality, person-centred assessments that support older Australians in accessing appropriate aged care services
- Performing face-to-face needs assessments requiring travel within the Loddon Mallee and or remote assessments via telehealth
- Providing clinical expertise and guidance to Non-Clinical Aged Care Needs Assessors
- Ensuring assessments are accurate, holistic, and aligned with relevant aged care standards and policies
- Undertaking other functions such as delegation and triage
