Therapist

• Provides direct patient care activities to home care or home health patients according to physician orders in the home environment including assessment, evaluation, treatment planning, implementation and instruction/supervision of physical therapy practice.
• Instructs and assists patient to perform various physical activities and in use of assistive and supportive devices such as crutches, canes, and prostheses.
• Instructs patient and family in treatment procedures.
• Re-evaluates patient and adapts treatment program accordingly to achieve maximum benefit.
• Confers with physician and other practitioners to obtain additional information, suggest revisions in treatment plan and to integrate physical therapy treatment with other aspects of patients.

Home Health Therapist

• Assists one or more Physical Therapists in evaluating and providing treatment to help patients recover from injury, disease, or surgery in a home care setting
• Prepares patients and equipment for therapy and implements treatment plans specified by the therapist
• Instructs the patient and guides them in exercises and stretching
• Provides hands-on therapy and treatments
• Records patients' reaction to treatment and provides status reports at the end of each treatment

Physical Therapist Assistant, Home Care

Occupational Therapist Assistant, Home Care

• Evaluates and provides initial treatment with patients recovering from injury, disease, or surgery in a home care environment.
• Establishes treatment plans using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility.
• Works cooperatively with physicians, case managers and treating therapists.
• Monitors, documents, and reports patients' progress and communicates needed health information regarding the patient to all appropriate health care team members.
