About the role
Key responsibilities & impact- Conduct telehealth evaluations for Medicare beneficiaries experiencing urgent, non-emergent clinical concerns
- Evaluate symptoms, review medical histories, and determine appropriate interventions
- Determine when escalation to the member's primary care provider, emergency department, or 911 is clinically indicated
- Document all encounters accurately and timely in the electronic medical record
- Assist in reducing avoidable emergency department visits and hospitalizations through proactive clinical intervention
- Provide patient education on disease management, medications, lifestyle modifications, and symptom monitoring
- Promote medication adherence and self-management of chronic conditions
- Provide clinical guidance and education to nursing staff on appropriate escalation pathways
- Participate in clinical case reviews and quality improvement initiatives
Requirements
What you’ll need- Active, unrestricted Nurse Practitioner license; Georgia preferred
- National certification appropriate to specialty
- Minimum of 3 years of clinical practice experience, preferably in primary care, internal medicine, family medicine, geriatrics, urgent care, or chronic disease management
- Experience managing hypertension and common chronic conditions, including diabetes, heart failure, COPD, and chronic kidney disease
- Prior telehealth or virtual care experience
- Strong clinical assessment and decision-making skills
- Excellent verbal communication and patient education abilities
- Familiarity with eClinicalWorks (eCW) or similar EMRs
