About the role

Key responsibilities & impact
  • Conduct comprehensive telehealth intake assessments, including clinical screenings and information gathering, to determine client needs and appropriate program placement
  • Coordinate closely with Admissions, Business Development, and Telehealth Clinical teams to ensure a smooth transition from intake through treatment initiation
  • Apply professional judgment to review client assessments and assist in identifying appropriate services, treatment recommendations, and care pathways
  • Serve as a key point of contact for internal teams to support communication and coordination of telehealth services
  • Provide clients with education on program expectations, treatment processes, and available services
  • Conduct scheduled individual check-ins or supportive sessions to monitor progress, address barriers to care, and reinforce treatment goals
  • Assist therapists with treatment planning, client assignments, and coordination of services within the telehealth program
  • Promote client engagement, accountability, and retention throughout the treatment experience
  • Maintain accurate, timely, and complete documentation related to client intakes, care coordination activities, and treatment updates within required systems
  • Ensure all documentation complies with organizational policies, regulatory requirements, and confidentiality standards
  • Communicate updates and client progress with internal teams through systems such as Salesforce and the electronic medical record
  • Participate in team meetings, case reviews, and program initiatives to support the ongoing development of telehealth services

Requirements

What you’ll need
  • Associate's degree or higher in Social Work, Psychology, or another related Human Services field
  • Two to three years of work experience in behavioral health, mental health, and/or substance use disorder treatment settings, preferred
  • Adherence to the Healthcare Code of Ethics and professional standards of conduct
  • Knowledge of state and federal confidentiality regulations
  • Familiarity with Joint Commission standards and documentation expectations
  • Strong organizational, communication, and clinical support skills
  • Ability to work effectively as part of a multidisciplinary treatment team
