About the role

Key responsibilities & impact
  • Connect Banner Plan Network Members with services to maximize health care benefits
  • Coordinate services into or out of care settings
  • Assist with patient navigation through the healthcare system

Requirements

What you’ll need
  • Associates Degree or equivalent working knowledge in community relations
  • Minimum of two years experience in community relations or healthcare related field
  • Reliable transportation and valid driver's license
  • Good oral and written communication skills
  • Proficiency with Office Suite
  • Ability to prioritize and make sound decisions
  • Ability to work cohesively in a team environment
  • Community Health Worker or Medical Assistant certification preferred
  • One year experience with Medicaid, Commercial Insurance and/or Medicare preferred
  • Bilingual skills (English/Spanish) preferred
