About the role
- Care transition management and provision
- Care coordination and assessment of care needs
- Training of patients, relatives and nursing services in the patient’s home
- Handling of infusion pumps in accordance with the Medical Devices Act (MPG)
- Supporting continuity and reliability of care
- Patient monitoring, performance documentation and regular feedback to therapy leads

Requirements
- Priority 1: Registered nurse (m/f/d)
- Pediatric nurses are also welcome
- Priority 2: Strong communication skills, empathy, resilience and team spirit
- Driver’s license and IT/computer skills required
- Priority 3: Willingness to travel on day trips
