About the role

Key responsibilities & impact
  • Accountable for working in partnership with facilities, discharge planners, clinic staff and healthcare workers to promote home healthcare, hospice and Care Transitions.
  • Builds relationships with referral sources to provide education on what Avera can offer their patients.
  • Provides education to patients, families and facilities on what home health, hospice and Care Transitions programs are.
  • Functions as a liaison between health care facilities, clinics, and physicians in the coordination of discharge planning needs for homecare services.
  • Initiates referral process and interdisciplinary communication.

Requirements

What you’ll need
  • Drivers License - Licensing Board Upon Hire.
  • Bachelor's in Healthcare, Sales, or Business.
  • Training and experience in health care related field, home health or hospice.
