About the role

Key responsibilities & impact

Accountable for working in partnership with facilities, discharge planners, clinic staff and healthcare workers to promote home healthcare, hospice and Care Transitions.
Builds relationships with referral sources to provide education on what Avera can offer their patients.
Provides education to patients, families and facilities on what home health, hospice and Care Transitions programs are.
Functions as a liaison between health care facilities, clinics, and physicians in the coordination of discharge planning needs for homecare services.
Initiates referral process and interdisciplinary communication.

Requirements

What you’ll need

Drivers License - Licensing Board Upon Hire.
Bachelor's in Healthcare, Sales, or Business.
Training and experience in health care related field, home health or hospice.
