About the role
Key responsibilities & impact- Accountable for working in partnership with facilities, discharge planners, clinic staff and healthcare workers to promote home healthcare, hospice and Care Transitions.
- Builds relationships with referral sources to provide education on what Avera can offer their patients.
- Provides education to patients, families and facilities on what home health, hospice and Care Transitions programs are.
- Functions as a liaison between health care facilities, clinics, and physicians in the coordination of discharge planning needs for homecare services.
- Initiates referral process and interdisciplinary communication.
Requirements
What you’ll need- Drivers License - Licensing Board Upon Hire.
- Bachelor's in Healthcare, Sales, or Business.
- Training and experience in health care related field, home health or hospice.
