About the role
Key responsibilities & impact- Coordinate care for assigned clients and establish goal-directed care plans from admission through discharge
- Perform on-site supervisory visits to assess clients, families, environments, and clinical caregivers, and complete follow-up documentation
- Ensure availability and proper operation of patient-care equipment and supplies
- Provide direct client care as needed
- Manage expectations and satisfaction of internal and external customers
- Evaluate the quality and effectiveness of nursing practice and services, analyze data, and identify opportunities to improve services and patient outcomes
- Provide nursing updates and obtain re-authorization for continued care
- Supervise, orient, train, educate, and evaluate clinical field staff
- Identify professional practice strengths and development areas
- Contribute to nursing education and professional development of staff, students, and colleagues
- Participate in hiring and termination decisions affecting nursing staff, as appropriate
- Maintain compliance with company policies, laws, regulations, and professional standards
- Participate in after-hours client-care call
- Promote quality and safe client care through QA-plan activities and metrics
Requirements
What you’ll need- Must have good standing license (RN) in the state in which the clinician will practice
- Must have at least 2 years of hands-on nursing experience as an RN
- Must be able to travel to patients’ homes in designated territory
- Must have reliable transportation, valid driver’s license, and pass MVR check
- Current CPR certification from AHA or ARC
- Must be willing to supervise nurses providing in home skilled nursing care to infants, children, adolescents and/or adults
- Must be able to speak, write, read and understand English
- Occasional lifting, carrying, pushing and pulling of 25 pounds
- Must be able to lift 50 pounds
- Prolonged walking, standing, bending, kneeling, reaching, twisting
- Must be able to sit and climb stairs
- Must have visual and hearing acuity
- Must have strong sense of smell and touch
- Must be able to sufficiently reposition patients and move equipment without assistance
- Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
- Occasional rapid movement and physical agility in response to the spontaneity of children and those patients with neurological impairment
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids and infectious diseases
