About the role

Key responsibilities & impact

Coordinate care for assigned clients and establish goal-directed care plans from admission through discharge
Perform on-site supervisory visits to assess clients, families, environments, and clinical caregivers, and complete follow-up documentation
Ensure availability and proper operation of patient-care equipment and supplies
Provide direct client care as needed
Manage expectations and satisfaction of internal and external customers
Evaluate the quality and effectiveness of nursing practice and services, analyze data, and identify opportunities to improve services and patient outcomes
Provide nursing updates and obtain re-authorization for continued care
Supervise, orient, train, educate, and evaluate clinical field staff
Identify professional practice strengths and development areas
Contribute to nursing education and professional development of staff, students, and colleagues
Participate in hiring and termination decisions affecting nursing staff, as appropriate
Maintain compliance with company policies, laws, regulations, and professional standards
Participate in after-hours client-care call
Promote quality and safe client care through QA-plan activities and metrics

Requirements

What you’ll need

Must have good standing license (RN) in the state in which the clinician will practice
Must have at least 2 years of hands-on nursing experience as an RN
Must be able to travel to patients’ homes in designated territory
Must have reliable transportation, valid driver’s license, and pass MVR check
Current CPR certification from AHA or ARC
Must be willing to supervise nurses providing in home skilled nursing care to infants, children, adolescents and/or adults
Must be able to speak, write, read and understand English
Occasional lifting, carrying, pushing and pulling of 25 pounds
Must be able to lift 50 pounds
Prolonged walking, standing, bending, kneeling, reaching, twisting
Must be able to sit and climb stairs
Must have visual and hearing acuity
Must have strong sense of smell and touch
Must be able to sufficiently reposition patients and move equipment without assistance
Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
Occasional rapid movement and physical agility in response to the spontaneity of children and those patients with neurological impairment
Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
Possible exposure to blood, bodily fluids and infectious diseases
