About the role
Key responsibilities & impact- Develop and oversee care plans from admission to discharge
- Conduct in-home supervisory visits and provide direct care as needed
- Ensure timely documentation and communicate care updates
- Train, mentor, and evaluate clinical staff
- Participate in hiring and performance management
- Support a positive and growth-oriented team environment
- Ensure regulatory and accreditation compliance, including CHAP
- Monitor service quality and implement improvements
- Promote safety and ethical care practices
- Participate in the on-call rotation
- Collaborate with care teams and external partners
- Maintain equipment and medical supplies
Requirements
What you’ll need- Active RN license in the applicable state
- Associate’s, Diploma, or Bachelor’s in Nursing
- Valid driver’s license and acceptable MVR
- Clinical experience per state/federal guidelines
- Ability to lift up to 40 lbs; walk, bend, kneel, and climb stairs
- Ability to work in varied home settings, with potential exposure to allergens and infectious diseases
- Must meet required health and vaccination standards, subject to applicable medical or religious exemptions
- Preferred: Home health or private duty leadership experience
- Strong clinical judgment and communication
- Leadership and team development skills
- Knowledge of home care regulations and documentation
