About the role

Key responsibilities & impact

Manage patient health needs as part of an interdisciplinary team through virtual primary care
Assess, diagnose, and create treatment and management plans appropriate for age, patient acuity, and clinical condition via virtual health modalities
Provide scheduled virtual visits
Perform dedicated asynchronous In-Basket cross-coverage for PCPs who are out
Participate in In-Basket work, workflows, and coverage for team members who are out
Safely triage and manage unfamiliar patients in a virtual setting
Navigate electronic health records and pivot between scheduled video visits and asynchronous chart review

Requirements

What you’ll need

Nurse Practitioner: Master’s or Doctorate’s degree from an accredited Nursing Program
Physician Assistant: Bachelor’s or Master’s degree from an accredited PA Program
Nurse Practitioner: Active RN and APNP license in the State of Wisconsin
Nurse Practitioner: Active board certification as an NP in Adult, Adult/Gerontology Primary Care, or Family
Physician Assistant: Active PA license in the State of Wisconsin
Physician Assistant: Active PA board certification
Active DEA license or ability to apply for such license prior to or upon hire
CPR certification and advanced life support certifications if relevant to the practice area (ACLS, PALS, NRP)
Proficiency navigating electronic health records
Strong independent clinical judgment
Ability to safely triage and manage unfamiliar patients in a virtual setting
Adaptability between scheduled video visits and asynchronous chart review
Work schedule: 100% FTE, Monday–Friday, between 8:00 AM–5:00 PM; no holidays or call
