About the role
Key responsibilities & impact- Manage patient health needs as part of an interdisciplinary team through virtual primary care
- Assess, diagnose, and create treatment and management plans appropriate for age, patient acuity, and clinical condition via virtual health modalities
- Provide scheduled virtual visits
- Perform dedicated asynchronous In-Basket cross-coverage for PCPs who are out
- Participate in In-Basket work, workflows, and coverage for team members who are out
- Safely triage and manage unfamiliar patients in a virtual setting
- Navigate electronic health records and pivot between scheduled video visits and asynchronous chart review
Requirements
What you’ll need- Nurse Practitioner: Master’s or Doctorate’s degree from an accredited Nursing Program
- Physician Assistant: Bachelor’s or Master’s degree from an accredited PA Program
- Nurse Practitioner: Active RN and APNP license in the State of Wisconsin
- Nurse Practitioner: Active board certification as an NP in Adult, Adult/Gerontology Primary Care, or Family
- Physician Assistant: Active PA license in the State of Wisconsin
- Physician Assistant: Active PA board certification
- Active DEA license or ability to apply for such license prior to or upon hire
- CPR certification and advanced life support certifications if relevant to the practice area (ACLS, PALS, NRP)
- Proficiency navigating electronic health records
- Strong independent clinical judgment
- Ability to safely triage and manage unfamiliar patients in a virtual setting
- Adaptability between scheduled video visits and asynchronous chart review
- Work schedule: 100% FTE, Monday–Friday, between 8:00 AM–5:00 PM; no holidays or call
