About the role
Key responsibilities & impact- Provide comprehensive cancer care for patients and caregivers throughout all phases of cancer care
- Assess barriers to care, psychosocial needs, and learning needs
- Advocate for and support patients and caregivers collaboratively with teams and support services
- Facilitate timely and coordinated transitions of care
- Provide anticipatory guidance and reinforce patient and caregiver education
- Guide patients and caregivers through shared decision-making
- Provide mental and emotional support throughout the continuum of cancer care
- Participate in multidisciplinary tumor board meetings
- Participate in continuous quality and process improvement initiatives
Requirements
What you’ll need- Bachelor's Degree in Nursing (BSN) required
- 3 years of relevant clinical nursing experience required
- Registered Nurse license in the state of Wisconsin required upon hire
- Basic Life Support/CPR required upon hire
- Master's Degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field preferred
- 1 year of relevant experience in the specific patient population being supported preferred
- Applicable oncology certification based on patient population preferred
